Since the release of the Institute of Medicine (IOM) Report ‘To Err is Human’ in 1999, the Agency for Healthcare Research and Quality (AHRQ) and the Department of Defense (DoD) have been Federal leaders in the patient safety movement in the United States. “A major focus of these agencies has been supporting research and development activities centered on improving team performance in the delivery of care.”

One outcome of many years of Research related to Teamwork, team Training, and culture Change was the systematic Approach ’TeamSTEPPS’: Team Strategies and Tools to Enhance Performance and Patient Safety.

Competencies that “served as the foundation for the TeamSTEPPS Initiative” are: Team leadership, Mutual Performance Monitoring, Backup behavior, Adaptability, Team/collective orientation, Shared mental models, Mutual trust, Closed-loop communication.
